Systemic Histoplasmosis with Pericadial Effusion


Abstract
A case of Systernic Histoplasmosis is reported and briefly reviewed.
The patient was 16 year old Korean girl admitted to the 11 th Evacuation Hospital complaing of epigastric fullness, dysprea, orthopr_e3 and facial puffi.ress for the 3 days prior to admission. Physical examination revealed generalized edema, marked hepato-splenomegaiy, quiet pre--ordium and a shift of cardiac apex to the left. Her course was marked by an undulent fever and stigmata of congestive heart failure.
Laboratory data disclosed depression of formed elements in the blood.
Pericardiocentesis was performed repeatedly with aspiration of pus only at the time of the initial procedure. Subsequent aspirations were productive of only serosanguinous fluid.
Repeated culture of her blood failed to grow any causative organism despite the patient¢¥s undulent fever. Various antibiotics regimens were attempted but failed, and the patient expired after 72 days of hospitalization.
Postmortem spezimens were obtained from spleen, liver and heart using the Vim Silverman Biopsy Needle, and histological examination of the spleen was interpreted as consistent with Histoplasmosis.
